This information page is for students who have applied to study in 2012 and have received an enrolment pack.  These are the documents referred to in the enrolment checklist  that you may need to download, fill out and return to us with all your other enrolment documents.

The School of Veterinary Nursing Study Guide is for students to assist with getting started and navigating their programme.  Enrolled students will receive a booklet. Please contact the School for further information.

Health and Safety Sheet for Workplacements  Are you safe to commence work at your assigned placement? Use this form to check the hazards in your workplace.

Code of Privacy for Workplace Training  Please sign and return this form with your enrolment information.

Expectations Contract This is a contract between yourself and the School of Veterinary Nursing which clearly outlines what you can expect from us and what we will expect from you to help us meet these expectations.

Student Responsibilities During Workplace Experience

(RPL) Recognition of Prior Learning Use this form if you have extensive previous experience in the veterinary nursing field; or have been credited one or more of the unit standards from another provider. 

Credit Transfer Use this form if you have successfully passed a course or module with another provider which covers all the elements outlined in the unit standard.

Arrangement for Alternative Assessments  Please read the information about our Disabilities Services first.  If you require an alternative assessment please use this form.

Risk Exposure  This outlines all risks you may encounter in your programme.  Please read and understand all of these risks before you start the programme.

Risk Management for Students with Allergies  If you have a known allergy which may be affected by your study in this programme you must complete this form to enable us to risk-manage your exposure.  During your study you may be exposed to any of the following:  latex, animal fur, antibiotics, pollens as well as other allergens.

Maori and Pacific Island Support Information for Maori and Pacific Island students.

Disabilities and Learning Centre Information about these services which are helpful if you need further assistance with your study.

Declaration of Tetanus Status  Evidence of your current Tetanus status from your doctor or G.P. is required for health and safety purposes. This must be submitted by the start date of your programme or you may be excluded from your first block course.

Computer Specification Requirements
You are required to have access to a suitable computer for study in our Animal Care / Veterinary Nursing/ Rural Animal Technology courses. This document outlines the recommended downloads and minimum requirements.
